You'll have to shoot in (M)anual exposure mode and put the SB-400 into Manual mode also (Custom Menu Setting e2), but I'd imagine that with the type of macro rig you're talking about, those settings should work. I don't know about the lens, but D90 will fire SB-400, even without a lens attached at all.
